Complaint Form

You can use the pro-forma complaints form if you wish.

Alternatively, you can detail the complaint in a general form (including in bullet points).  

Whatever the form in which you wish to lodge a complaint, please address the following issues:

• What is the nature of your complaint against the Independent Commission Against Corruption and/or its officers?  Please identify the grounds for the complaint - is it:
  • Abuse of power?
  • Impropriety?
  • Other forms of misconduct? 
  •  Maladministration (see definitions provided)?
• Please provide the names, details, dates of the alleged conduct.
• Please advise whether you wish your identity to be kept confidential and why.
